Vehicle File Cover Sheet & Service Chart

Carriers must maintain the following records for each commercial motor vehicle (including trailers) that they control for 30 consecutive days: An identification of the vehicle including unit #, make, VIN, year, and tire size, as well as the person furnishing the vehicle (if not owned by the carrier); A means to indicate the nature and due date of various inspection and maintenance operations performed; A record of inspection, repairs, and maintenance indicating their nature and date; and A record of tests conducted on pushout windows, emergency doors, and emergency door marking lights on buses. This document is intended to satisfy these requirements and serves as a perfect cover sheet for each vehicle file. Pertinent Regulation(s): 49 C.F.R. 396.3.
