Initial Audit Questionnaire

To initiate your ACIG mock DOT audit, please complete the following questionnaire and upload the requested documents. Compliance Documents

Please upload copies of the following basic compliance documents:

Driver List

Upload list of all CMV drivers utilized by your company in the past 12 months (including any terminated drivers). Please provide their full names, DOB, whether they are CDL or non-CDL, hire dates, and term dates (as applicable).

Vehicle List

Upload list of all commercial vehicles (trucks & trailers weighing over 10,000 lbs. either singly or in combination) utilized by your company in the past 12 months (including any disposed of in past 6 months). Please provide the year, make, model, VIN, and unit number (if assigned).

Proof of Insurance

Upload proof of your commercial auto liability insurance (e.g., form MCS-90 or certificate of insurance).

MIS Report (if applicable)

If you utilize CDL drivers, upload an MIS report summarizing your drug/alcohol tests completed in 2024. The MIS report is a form published by the FMCSA and is required for regulated fleets. If you utilize a third-party to manage drug/alcohol testing (e.g., a consortium), they should be able to provide you this report.

Accident Register (3 Years)

Upload a copy of your DOT-recordable accident register listing all DOT-recordable accidents for the past 3 years. Please include date of accident, location, driver involved, whether the accident involved a fatality, injury, or towaway, and whether your driver received a citation for a moving violation.
