
Carriers who are using a driver for the first time or intermittently must obtain a signed statement from that driving that lists the total time the driver spent on-duty and driving during the immediately preceding 7 days, as well as the time at which the driver was last relieved of duty prior to beginning work for the carrier. A copy of the completed statement should be kept in the driver's file.

Inspections must be performed by an individual meeting the qualification requirements in 49 C.F.R. 396.19 & .25 (as applicable). A copy of the inspection report should be kept on the vehicle at all times.
Carriers that employ CDL drivers must run annual queries through their accounts with the FMCSA's Drug and Alcohol Clearinghouse to determine whether any drug/alcohol violation information exists in the Clearinghouse on each driver. The regulations require that carriers obtain written consent from their drivers to run these queries. This document can be used for that purpose.
