
As a prospective employer of a CMV driver, you are required to request specific safety performance history and drug/alcohol testing violation information from any DOT-regulated employer for whom the applicant worked within the 3 years prior to his/her application with you. Applicants must provide written consent for these requests, and the consents must accompany the requests. This document can be used for these purposes, and copies should be kept in the drivers' files.

Learn about Compliance+Carriers must maintain an accident register that lists all “DOT recordable accidents” occurring within the prior 3 years. A “DOT recordable accident” is one that involves (i) a fatality; (ii) bodily injury to a person who, as a result of the injury, immediately receives medical treatment away from the scene; or (iii) disabling damage to any vehicle that requires that motor vehicle to be towed away from the scene. This document is intended to satisfy this requirement.
Carriers must annually review each driver’s motor vehicle record as well as other information described in 49 C.F.R. 391.25, and complete an Annual Review form to place in the driver’s file. This document serves as the Annual Review of Driving Record.
Carriers are responsible for ensuring that any individuals performing annual inspections under 49 C.F.R. 396.19 are qualified to do so. This form should be completed and retained by the carrier for any such individual.
